Hi, I'm currently working on redesigning our performance management paperwork for the regular one to ones and end of year reviews/appraisals. Does anyone have paperwork that works really well and that you'd be prepared to share with me? Or does anyone know of someone who designs forms in the Bristol area?

I have designed it in many different organisations, from the simple and easy MBO to the sophisticated engineer approach with formulas and graphs. Of course, you should adapt the right one to the culture and to specific needs.
My suggestion is to keep it simple simple and work on an MBO approach only, mostly if this is the first implementation.
In my company this year we are implementing a Competence Assessment too, in the same document and process.

We've just redesigned ours and actually simplified them so that the emphasis is more on the conversation. We've renamed Personal Performance Planner as we found that the words 'reviews/appraisals seemed to have negative vibes to some staff,especially if they have come from other companies where the reviews were used as a 'whipping tool'.
To do this, I actually went onto google, looked at a few templates, then with the help of the Line Managers input, designed them to suit our needs and I hope to use these as much for succession planning as well as performance issues.

I have personally found that this plan and a few other things, which I then adapt to suit our business on http://employeeperformancesolutions.com/ - but I only use this for ideas that are then adapted to our company and the legal side of how we do things.
