I’ve nearly completed the first assessment of level 5 DVP, just have my CPD plan to do. I‘ve watched the video but I’m still not sure if I have to use my SWOT for all 4 entries or can I use 2 from the SWOT weaknesses and 2 which are related to my HR role at work as I have projects on the go and learning needs from them ?
A response would be much appreciated.

Your assessor will be looking for you to reflect on your development needs which are likely to carry forward from Activity 1 but do not necessarily have to. You can do more than 4 entries as well, this is more of a minimum to show how you approach assessing options to develop yourself.
