In our company we have a combined format document, which incorporates the Leaving Interview questions and a tick box to cover the leaving considerations.
So, our form has the usual leavers questions etc and then has tick boxes for the following:
Equipment:
* Mobile Phone returned
* Lap top returned
* Door fob returned
* Other.....
Access:
* Email account suspended - diverted to colleague. Name of colleague........
* Intranet access removed
* Company drive folder access stopped
* Name removed from email groups
Documents:
* Name removed from public documents
* Handover received. Name of recipient.........
I'm sure there will be much more than we could document, but we have put the main bulk of the requirements on this form.
