I've been tasked with looking into business forms for individual training requests and I was wondering if anyone had any advice?
We need to capture the usual items about the what, how and ROI but I'm concerned with it being too complex with lots of questions putting people off filling them out.
Are training forms common in your organisations, or have you a different way of capturing and reviewing training requests?

This is a great question. I'm wondering how these forms will be used. Will the individual needing the training complete them and submit them straight to L&D? Or will the individual send them via their line manager? Or perhaps they fill it in together?
How do you see these forms being used in your organisation?
I'd love to hear more.
