    We have a series of questions that we sit and talk through with the leaver, and they cover things like what they have enjoyed most and least, what improvements they think can be made from a business perspective, what their opinions are of the company benefits and rewards, we ask them to give feedback on how effective their line manager has been and then ask them for any other feedback. I finally ask them if they are OK with me talking through their feedback with their Manager.

    What I then do with the information is talk it through with the leavers manager if they approve my doing so, and then I will collate a quarterly report to feedback to the remainder of the executive team and then work with areas where failings are being highlighted or likewise congratulate areas that are praised.

    In terms of who conducts them, I do from the HR perspective and it means its on neutral ground and wouldn't be influenced by the manager.

    I would be interested as well in the structure of the exit interviews, questions, data consolidation - I could share mine if you want!

    I am looking to improve it, so any ideas would be welcome!

    We have created a form which we ask leavers to complete. The form includes what they most enjoyed, least enjoyed and any suggestions they have.  We also ask them what they think about their manager and the morale within the department.  We meet with each leaver once they have completed the form so that we can get more in depth information.  We then collate this information and provide the stats and the monthly management board meeting and look to see if there are any trends.

    I started using SurveyMonkey recently for our exit interviews and have a mix of questions requiring either scaling/rating or free text responses.  This is easier to then report on as you can easily collate the responses and calculate the ratings, etc.  (I used to just use Excel but that would take ages to collate and report on!)

    I tend to do a Management report every six months or so (or when I have lots of data to use); I don't always give all the information, but will try to give an overview of reasons for leaving, general percentages (i.e. how many leavers would recommend us as somewhere to work), in addition to some free text responses that I feel are important for the Senior Managers to hear (either good or bad!).  

    I tend to use the responses to exit interviews (and my subsequent management reports) as backup for introducing new incentives or management policies, etc.
